In what cases is sinus lift treatment necessary?

Based on the X-rays and specialized CT scans of the jawbone, your maxillofacial surgeon may suggest a sinus lift surgery treatment plan for you before starting the implant treatment to prepare the area for implant placement.

The sinuses are located in the left and the right sides of the upper jaw. The bone beneath the sinus is relatively low in density (porous), and after the extraction of the posterior teeth in the upper jaw, a gradual process of bone and gum tissue resorption begins.

During the process of tooth extraction, the sinuses may increase in volume or descend downward, occupying a larger residual space.

How is sinus lift surgery performed?

During the sinus lift surgery, the surgeon lifts the sinus and places an implant with a suitable length, diameter, and thicker for increased stability in the edentulous area. They fill the sinus cavity and the surrounding bone with autografts or allografts to strengthen the bone and perform vertical bone augmentation in the posterior maxillary region to provide a better foundation for dental implants.

Implants can be placed immediately during or after sinus lift surgery, or with a delay. If decision is made to place implants without performing sinus lift surgery in such conditions, the sinus will undoubtedly be perforated or damaged, leading to acute and chronic infections and additional surgical procedures to close the sinus.

Sinus Preparation Duration for Dental Implant

After sinus lift surgery and reconstruction of the sinus volume and its bone, the surgeon may be able to perform implant placement simultaneously with sinus surgery, depending on the type of surgery and the type of bone graft.

Alternatively, the surgery may be performed in two stages. In the first session, the sinus surgery and bone reconstruction are performed, and it takes about three months for the graft to integrate. After this period, if the sinus and bone area are prepared, the implant surgery is performed.

Depending on the edentulous area and sinus conditions, the type of surgery and the duration required for post-surgery preparation of the space for the implant usually vary among different individuals. However, in most cases, it takes about three to five months after sinus surgery so that the soft and hard tissues of that area are ready for implant placement.

Types of Sinus Lift

The treatment process for open sinus lift involves external access to the maxillary sinus through the lateral aspect of the alveolar ridge. The Schneiderian membrane is lifted, and the resulting space is filled with a bone graft or a bone substitute.

The treatment process for closed sinus lift is performed through the implant bed and cavity. This procedure is carried out simultaneously with implant placement. After creating the cavity and preparing it for implant placement, the Schneiderian membrane is elevated in a crestal transposition manner, i.e. it is lifted using osteotomes, and the resulting space is filled with a bone graft or bone powder. Following that, an implant with a suitable length and diameter for the cavity is placed. The closed sinus lift method can significantly increase vertical bone height and elevate the sinus space to avoid damage and pressure.
